Invoices must include dealer name and address, Consumer name and address, installation date, installed serial numbers and model numbers and purchase price.

If you are submitting a claim as part of a special consumer category, evidence of eligibility is required. Examples of accepted documentation to prove eligibility include, but are not limited to, redacted paystub, employee ID, employment verification letter, news article covering natural disaster.
