Submit your incentive information and attach proof of sale to claim your incentive. Use your smartphone or tablet to capture your receipts if you are applying using either of those devices, or scan and attach copies when doing so via laptop or desktop.
You will need the email address you supplied when you submitted your incentive request and your incentive number from your confirmation email to get a status update.
